Service Coordination, Case Management and Intensive Clinical Case Management:
Supports are provided consistent with an Individualized Support Plan, Plan of Care and/or Treatment Plan. The plans provide a clear understanding of a service recipient’s service needs regarding clinical treatment issues, skill acquisition, and a place to live, socialize, work, and develop relationships.
This support provides a variety of activities that emphasize relationships, pre-vocational training, therapeutic recreation, behavioral supports and training in skill acquisition.
Vocational & Rehabilitation Training:
Support is provided to service recipients in a variety of settings, including the Hunt Opportunity Center, community settings, and enclaves. Many individuals work in the community through the Supportive Employment program.
