Medical, Mental Health, Behavioral and Psycho-social Rehabilitation Services:

Bio-psychosocial model of care is provided. On-site staff physicians provide primary health care services or these services can be secured in the community. Specialized clinical psychosocial services & supports are also provided to service recipients with significant psychiatric and behavioral needs. Outpatient mental health services are provided through the Clinic and other Agency components. Clinical support-staff include physicians, nurses, PT, OT, SLP, Registered Dietician, psychiatrist, clinical psychologist, behavior analyst and other licensed and certified mental health professionals.

Nursing and Personal Assistance:

Services are offered in the person’s home and provide medical and/or personal care staff to help support the service recipient in his or her community, provide a more positive quality of life, allow greater normalization and prevent institutionalization.
